- Names and Dates of birth of Parents and Spouse
- Personal and financial details (Bank Statement) of person paying for trip
- Full name, address and contact information, including e-mail address of the person in the USA with whom you are staying
- Invitation letter (Notarized with copy of citizenship)
- Full name, address and contact information, including e-mail address of immediate family members in the USA.
- Intended dates of travel
- Children under 14 yrs: Notarized consent forms from both parents, Parents valid visa, letter from school
